since mettaton is a robot, he is immune to the disease, right? napstablook is made of magic, so they probably died too... but they were already living in isolation so maybe...?

Yeah, that's an admitted plot hole. I didn't feel like including them or Flowey in this story because I didn't think they would contribute anything relevant to the overall plot. This story has enough characters as is, and I didn't visualize including them in any meaningful way since the story would get too scattershot.
I'd say yes, honestly. With no one to manage Mettaton's body, he'd break down and die. Napstablook died because he was in the Ruins during the pandemic, and Flowey? Well, I think Flowey could've died too. Like I said, that was a plot hole that I didn't address and I apologize for that.
