The boy turned, having heard the voice of One For All, and rushed to help.
Eight smiled - a gruesome thing, or so Mirai would say - and looked upon the boy as he spoke in rushed words and curses, reddened eyes going wide with dried tears at the sight of his injury-
And- and all Eight could think at that moment was that everything would be alright.
“Have no fear…” His words weighed heavy in his heart, and he saw the boy’s mouth open in horror as his body convulsed in its death throes-
And with one last surge of One For All…
He sunk his bloody fingers into the boy’s mouth-
And smiled, as he felt the last wisps of his Quirk fade away into the boy – and his life with it.
All Might is dead, and the world doesn't know.
Izuku becomes the Ninth Holder of One For All... and he remains blissfully unaware of this for nearly a year.
He doesn't make it to UA.
He becomes a hero anyways.

In which Izuku receives a different quirk just before his fourth birthday. Sure, it was a really nice quirk and perfect for heroics, he just wished it didn't come with an arguably insane yet devout follower who worshipped the ground he walked on. She was a bit much.
