Love the idea of Sonic's multiple deaths stacking on his mental health. Sonic really do be dying and getting dragged back to fight for freedom of people who dont always want his help. Interesting to see Silver be the one to step in.
Kinda hope this fic continues to expand with Silver trying to save Sonic.

Loved the way Sonic was determined to get an answer from Shadow before making any changes to their relationship. (Could definitely a slow burn/misunderstanding trope version of this where Shadow tries to avoid answering to the point that Sonic straight up assumes shadow does hate him and attempts to walk out of Shadow's life) -

It was a square shape, a bit larger than a normal notebook, with a white ribbon twisted into a bow to keep it closed. Tails picked it up curiously. It was clearly Sonic’s as he’d never seen it before, so it would be an invasion of privacy to look inside. But then again, Sonic had left it out in the open; if it was private he could’ve at least put it in his room. However, he hadn’t expected Tails to be home anytime soon so-
Who was he kidding. He could argue with himself all day if he wanted, but in the end he knew curiosity would get the better of him anyway. Before he could stop himself, he pulled at the ribbon and opened the book.

Summary
After some investigation, Sonic discovers that he has Hanahaki disease. There is a plant growing inside his lungs through his throat. It will kill him if he leaves it alone. His other options are to confess his feelings, get a surgery to remove the plant, or lose his feelings.
He can't confess his feelings. They would only burden Shadow, and Sonic doesn't want that to happen.
He can't get the surgery, either. It would erase all the memories and feelings he has for Shadow (and for any other platonic love that was mixed into the romantic love this plant grew from).
His only option, then, is to lose his feelings for Shadow. That, unfortunately, is easier said than done.

He pressed his other hand into his forehead, using it as a makeshift headrest and squeezing his eyes shut. ‘Oh Chaos, he’s dead, I wasn't fast enough..’"

“It’s not that bad,” Sonic replied automatically, though the pounding in his head and still threatening nausea rather refuted the statement.
Knuckles snorted. “Sure, and you didn’t almost die of some computer infection and fall from space a couple hours ago.”
Sonic grimaced and tried to wave a hand in an unconcerned fashion against the words, but only succeeded in unbalancing himself and only stayed upright from the quick supporting hand on a shoulder.
The action did not, suffice to say, ease any tension.
