Summary
A few days after they returned to their 5-year-mission on the new Enterprise the crew encounter another Federation ship. The USS Ikarus. There shouldn't be another Federation vessel so far out in space and before the Enterprise can make contact the Ikarus starts firing at them.
The Enterprise gets away with little damage - except for Spock getting gravely injured during the attack - but the ship has several failures after that, for example the transporters aren't working.
Just like a ghost has possessed the Enterprise! -

During a geological survey mission, Spock suffers a traumatic brain injury caused by falling ricks. When he regains consciousness, the only person Spock recognizes and remembers is Bones. Leonard and Dr. M’Benga diagnose Spock with total amnesia caused by the TBI. As Spock’s mental condition deteriorates, he begins to descend into madness and violence. Jim must get him to New Vulcan before Spock’s mind is irrevocably destroyed.
