Summary
It's not III's job to explain the physiology to the families. Sometimes, the families don't even know why III is there. But as they cry and say the things they wished to say before their loved one had died, III is present, passing on the messages, and sometimes, passing some back.
Just one more word. One more I love you. III ignores the requests if they're not centered in love. They aren't being paid pennies to ask the recently departed to uncover the families secrets - they are there to soothe and support the transition. They're there to speak when the brain and body is holding on but the soul has decoupled from the physical. He holds their hands or their bodies close in the spirit realm, watching light come back to their eyes when they realize that it doesn't hurt anymore.
There is no such thing as pain in the spirit realm. It's a mortal concept. III stays as long as they need. And when they are ready, if they are ready, they lead the soul into the gentle hands of Death's reapers.

III’s body reacted before his brain could catch up. His hand finding Vessel’s chest almost automatically, then sliding down to his side, fingertips dragging over ribs, waist, hip.
A silent worship of the perfect figure above him.
Vessel’s hand came up to III’s mask, fingers hooking under the edge near his jaw. Fingers warm, slightly damp with sweat, and they tugged at the mask—pulling him closer, tilting his face up.
III moved his fingers fast in an inviting gesture towards Vessel; come on, bastard, come closer, lay on me, take me, devour me for all I care.
