The low growls were the first warning, always terrifying to hear. Harry could never quite shake off the fear they kindled, even though Draco had never hurt him.
Yes, he'd leave bruises wherever he gripped him, sometimes pulled on his hair enough to make Harry's scalp burn in places, bite him, though not hard enough to draw blood, and of course abused Harry's hole to the point Harry would need a lot of time to recover if not for magic, but he had never injured Harry.
Even at his wildest, even that first time when Harry feared Draco was lost forever, Draco hadn't caused him any real damage.

He hesitated to cast a spell, he didn’t want to hurt him. Draco growled louder, slamming his hands on the ground and revealing how his fingers and nails were extending, growing. He stared up at him with gritted teeth, suddenly much sharper, and hissed;
