It's a matter of perspective, tbh. Alya has a lot of scars, but they aren't noteworthy to her, because while others would think of, idk, getting a terrible burn as deeply traumatic, she just doesn't care. Not compared to shit that actually endangered her life and caused her to have nightmares for literal months. When she says that her skin is free of scars, she means free of the important scars that defined her life for about as long as she can remember
