Nightmare and his Crew 'recuit'(kidnap) some people to use as their 'pets' and pets can be a multitude of uses, from cleaning rooms and the grounds to helping with heats. Each one has a certain branding to their pets and take care of them really good despite needing to break them either a little or a lot so they never run away from the crew.
If already broken, they like to keep them and help them actually get a personality, they like breaking people but not to not having a personality, just that they can never go home because often than not, the pets come from universes that were destroyed by Error for the Multiverses' Safety.

Nightmare would be the first to say he wasn’t an expert on medical issues or how to help those that suffered from chronic illnesses. Sure, he himself had residual pain around his skull from the incident that took his right eye as well as being obviously blind on his right side, but it was different. He was managing himself, not others. He didn’t think he’d ever have to look after another living being with chronic pain and illness.
TLTR: how Nightmare learnt of all his boys medical issues and how he helped them overcome them or learn to live with them
